Volunteering is a fun and rewarding experience. It is a great way to meet new people, to learn new skills, and a way to give back
to the community. Volunteers are needed and appreciated by staff, patients, and visitors at Putnam Hospital.

Note: You will need to download and have two documents signed to submit your application. Both require your signature and a parent/guardian signature. You will upload them at the end of the application

Please read and review the following steps with your parents.
1. Complete application below, including uploading the completed Junior Volunteer - Parent form and the Marketing release. Schedule your interview. We prefer to speak to student volunteer applicant directly to schedule interviews.
2. Your medical clearance will be conducted by Employee Health. Medical requirements must be met before you can begin volunteering. Volunteers will need to provide proof that they have immunity or proof of vaccination for the following.
- Measles, Mumps, Rubella (MMR 1 and MMR 2)
- Varicella
- Tetanus, Diphtheria, and Pertussis (TDaP)
- Quantiferon Gold (TB)
- Covid-19
All volunteers who are active at PH during the flu season must have proof of the influenza vaccine or receive the vaccine at PH.
3. Obtain working papers obtained from your High School guidance office. Or provide proof
4. Download the Reference Form and supply two references.
5. Download and complete the High School Information Form .
7. Attend an orientation program and complete assignment training.
8. Call in when ill or unable to volunteer.
9. Junior Volunteers may start at 16 years of age. Junior Volunteers may not transport patients until they have 20 hours of volunteer experience.
10. We do require a minimum of 50 hours over the year. Please consider this obligation before completing the application process. To continue in the program we do require the 50 hours annually. We do not provide letters of recommendation for students until they have completed 25 hours of service. This shows a commitment to our program. Thank you.
